FACE provides culturally responsive health support for individuals living with long-term and chronic conditions, including HIV, diabetes, hypertension, chronic kidney disease, mental health conditions, and disability-related health needs.

We support people to understand their conditions, access appropriate care, and manage their health with dignity, confidence, and continuity.

Training & Capacity Building

We deliver education and training programmes that promote prevention, early intervention, screening, and self-management.
