We provide integrated, culturally responsive, community support for people living with long-term and chronic health conditions, including HIV, diabetes, hypertension, chronic kidney disease, and mental health challenges.
FACE exists to reduce health inequalities, improve access to services, and empower individuals and communities through prevention, education, early intervention, advocacy, and compassionate support. We work in partnership with communities, the NHS, and the voluntary sector to ensure that no one is left behind because of their background, culture, or migration experience.

- To promote prevention, early intervention, screening, and self-management through health education and awareness.
- To tackle health disparities by addressing cultural, structural, and social barriers, including stigma, language, access, and migration-related challenges.
- To strengthen community capacity through engagement, peer support, advocacy, and partnership working with the NHS, local authorities, and the voluntary sector.
